description Background: Neonatal sepsis is an important cause of morbidity and mortality in India. Neonatal health programmes such as the Essential Newborn Care Package focus on preventative and curative care for the reduction of neonatal sepsis. However, neonates continue to die as a consequence of sepsis, many of which deaths are preventable. This critical review examines the factors that impact on neonatal sepsis and evaluates the effectiveness of this package aimed at preventing neonatal death. Aims: To evaluate the effectiveness of the Essential Newborn Care Package in reducing neonatal sepsis in India. Methods: Principles from the methodologies of the systematic and literature approach were adopted for the identification of relevant research. A critical appraisal tool was applied to identified studies to assess quality and reliability. Findings elicited were analyzed, with subsequent emerging themes identified for discussion in accordance with the organizational, economic, socio-cultural and political context of India. Findings: The key themes identified as barriers for the uptake of interventions included in the Essential Newborn Care Package were; access, skills of care provider, increased, risk factors and health seeking behaviour. Recommendations for future practice are the development of policy, outlining clear delineation of health care workers roles and the responsibilities of services for the provision of a care continuum. Development of nursing practice requires research to identify and evaluate socio-cultural, environmental and behavioral variables which influence neonatal health for the development of operational strategies that exhibit socio-cultural sensitivity. Findings are consistent with priorities in nursing practice which emphasis recognition of cultural influences of care. Conclusion: Although the Essential Newborn Care Package is recognised as having had some impact on reducing neonatal sepsis in India, the conclusion of this critical review is that mechanisms employed for the implementation of the Essential Newborn Care Package were inappropriate for the context of India, Thus undermining programme efficacy.
