description Background of study: Tobacco smoking continue to be the leading causes of premature death in Malaysia for the last 3 decades. The national prevalence of smoking among the young adult is low compare to other age group. However, the percentage of quit attempt among young adult are among the highest. The aim of this study is to determine the prevalence and pattern of various stages of smoking cessation among college students using the Transtheoretical model (TTM) in order to provide basis for stage based intervention. Methods: A cross sectional survey was conducted among 2272 college students at 10 community college in Selangor between December 2017 and January 2018 using probability proportional to size sampling method. Data was collected using self-administered pre-tested Transtheoretical model (TTM) questionnaire. Results: The prevalence of smoking among the college students was 18.9% (95% CI 17.3 – 20.6). Majority of them were at pre-contemplation stages (61.5%) whom have no intention to quit in the next six months. Only 7.2% and 6.1% of them at contemplation and preparation stage respectively (plant to quit smoking within the next six months and one month respectively). Those at the action stage already have made specific changes to their behavior consist of 21.3%. Of those only 3.88% already maintain quit smoking for more than 6 months (maintenance stage). Conclusions The prevalence of smoking among college are comparable to the national figures. But it is alarming that majority of them still have no intention to quit compare to about 13% intended to quit smoking. A specific stage based intervention is needed to mobilize these group of smokers towards making a quit attempt.
