Cervical cancer can lead to various problems, including a reduction in quality of life, self-efficacy in self-care, and a low level of optimism regarding recovery. The involvement of family support is important in the treatment process as it aids patients in their journey to recover from illness. However, Indonesia currently lacks a structured and replicable model for family support to assist caregivers. This research aims to develop and determine the effectiveness of a Family-Centered Empowerment Model (FCEM) on quality of life, self-efficacy in self-care, and optimism in cervical cancer patients undergoing chemotherapy. Methodology: This research design used two phases, specifically qualitative and quantitative. The research's first phase was to develop an FCEM; the qualitative type used a narrative approach, and study participants in the qualitative and quantitative research were cancer patients undergoing chemotherapy and families of cervical cancer patients. The informants for qualitative research were eight cervical cancer patients and ten families of cervical cancer patients. The quantitative type used a quasi-experimental design; the second phase identified the effect of the model on quality of life, self-efficacy in self-care, and optimism for cervical cancer patients. The quantitative study gathered 106 participants and divided them into two groups of 53 individuals. FCEM was given to families of cervical cancer patients in the intervention group whose families had previously undergone a pre-test and were given 3 months for the families to apply the FCEM model to the patients. A post-test was conducted on the cervical cancer patients. The tools of qualitative research used interview guidelines developed by the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ), Self-Efficacy in Self-Care from the Symptom Management Self-Efficacy Scale (SMSES), and Optimism from the Life Orientation Test-Revised (LOTR), and the quantitative research used the same questionnaires. The quantitative statistical bivariate analysis used the Wilcoxon and Mann-Whitney tests. Results: The FCEM was qualitatively generated consisting of 13 elements; six elements were generated from the theme of quality of life, three elements were generated from self-efficacy and self-care, and four themes from optimism. The results of the Wilcoxon test in the intervention group before and after treatment showed significant differences in average scores and effect scores on the quality of life, self-efficacy in self-care, and optimism variables compared to the control group. From the results of the Mann-Whitney test between groups, a comparison of post-test scores between the intervention and control groups showed significant results in the average scores and effect scores on quality of life variables, self-efficacy in self-care, and optimism. Conclusions: The family-centered empowerment model resulted in 13 elements. This model was effective in improving quality of life, self-efficacy in self-care, and optimism among cervical cancer patients.
