Yet, no specific data were available regarding prevalence of overweight and obesity among employees in sub-urban state like Terengganu. Thus, this study aimed to determine the prevalence of overweight and obesity and to explore the relationship between socio-demographic factors, dietary intake, physical activity (PA) level, stress, anxiety and depression (DASS) level among employees in education sector in Kuala Nerus, Terengganu. A cross sectional study was conducted between March 2016 to January 2017 among employees from public university and schools around Kuala Nerus, Terengganu. Using a convenience sampling method, two schools, one higher educational institutes and one public university participated in the study. A self-administered questionnaire was used to obtain information on socio-demographics whilst data on dietary intake, PA level and DASS were collected using Food Frequency Questionnaire (FFQ), International Physical Activity Questionnaire (IPAQ) and Depression, Anxiety and Stress Scale (DASS-21), respectively. Height, weight and waist circumference were measured and Body Mass Index (BMI) was classified based on World Health Organization (WHO, 2000) cut-off points. Multinomial logistic regression was performed to identify the associated determinants of overweight and obesity. A total of 341 subjects with mean age of 37.2 (9.1) years old were involved in this study with comparable proportion of men (51.9 %, n=177) and women (48.1%, n=164). Majority of the participants were non-academic staff (77.4%) whilst academic staff accounted for 22.6%. The prevalence of overweight and obesity were 39.6% and 21.1%, respectively. Men, having a tertiary education level, married and no history of hypertension (HPT) was significantly associated with higher proportion of overweight and obesity (P < 0.05). There was no significant difference between BMI categories in mean total energy intake (P = 0.542), PA level (P = 0.406), stress level (P = 0.325), anxiety level (P = 0.807) and depression level (P = 0.785). Multinomial logistic regression analyses showed that history of HPT (RRR=5.63; 95% CI 1.59, 19.96) and high income level (RRR=3.76; 95% CI 1.69, 8.33) was 5.63 and 3.76 times as likely to be overweight, respectively. In contrast, compared to secondary education level, the chance of individuals with higher education level being overweight was 68% lower (RRR=0.32; 95% CI 0.19, 0.55). Meanwhile, individuals with history of HPT was 15.87 times as likely to be obese (RRR=15.87; 95% CI 4.50, 55.92) whilst individuals with higher education level (RRR=0.51; 95% CI 0.26, 1.00) had 49% less chance to be associated with obesity. The findings from this study noted a considerably high prevalence of overweight and obesity as compared to the national prevalence. Overweight/ obesity is an established risk factor of HPT, thus prompt interventions are needed to prevent the development of HPT and later complications. The direct link between high income level and overweight and the protective effect of high education level against both overweight and obesity should provide information for future interventions tailored to the needs of similar working population.
