It had a significant impact on the elderly's QoL which changed diet, mental and physical health. Thus, maintaining a good nutritional status and mental health is important. This study aimed to evaluate the nutritional status, mental health and associated factors on the QoL of the elderly in Terengganu, Malaysia during the endemic period and explore their living experiences during the pandemic. Methodology: Phase I was a cross sectional study, in which 130 community-dwelling elderly people aged 60 years and above living in Terengganu were recruited using stratified random sampling. Socio demographic, anthropometry, semi-structured food frequency questionnaire, Malnutrition Risk Screening Tool-Hospital (MRST-H), Mini Nutritional Assessment- Short Form (MNA-SF), Depression, Anxiety, and Stress Scale (DASS-21) and Short Form-36 Health Survey (SF-36) data were collected. Venous blood (3 ml) was obtained to measure C-Reactive Protein (CRP) level. Data were analysed using IBM-SPSS version 21.0. Simple and multiple linear regression analyses were performed to evaluate the association between associated factors and QoL. Phase II was a qualitative study conducted using a semi-structured interview regarding the living experiences during pandemic among ten participants that was purposively selected from Phase I. Results: Phase I study showed that approximately two-third out of 130 elderly were female, with majority of them were Malay (99.2%), and had chronic diseases (70.0%). The median age was 67.39 ± 5.71 years and mean body mass index (BMI) was 26.97 ± 4.89 kg/m2. The overall prevalence of elderly at risk of malnutrition according to MNA-SF and MRST-H was 12.3% and 13.8%, respectively. Participants demonstrated inadequate intake of energy, fiber, potassium, calcium, vitamin D, E and K compared to the Malaysian Recommended Nutrient Intake (RNI) (p<0.05), whereas exceeded the recommended amount (p<0.05) for protein, cholesterol, and vitamin A intake. Only a small percentage of the elderly suffered mild, moderate, severe and extremely severe stress (6.2%), anxiety (3.8%) and depression (5.4%). Out of 74 participants, 3.8% had high serum CRP concentration, which were likely contributed by those who had higher stress, anxiety and depression levels as compared to those with normal CRP concentration (p<0.05). The median score for total QoL was 75.63 (15.53). Age (p<0.001), gender (p=0.022), BMI (p=0.003) and depression level (p<0.001) were significant predictors for total QoL. Qualitative study showed the pandemic COVID 19 slightly negatively impacts on nutritional status, mental health and QoL. However, they were able to overcome these difficulties by following SOPs, practicing coping strategies and receiving support from family and others. Conclusion: The findings showed no significant negative effects on the nutritional status, mental health and QoL among community-dwelling elderly in this study two years after infection with COVID 19. However, increment of age, female, higher BMI and higher level of depression were associated with lower QoL. Thus, this study provides the basis for further investigation into promoting health and well-being of community-dwelling elderly in this country. In addition, healthcare professionals can advocate healthy lifestyle approaches such as nutrition, exercise, and mental management as BMI and depression level are the two modifiable predictors of QoL.
