It continues to pose a significant problem to health care systems across the world, not only causing harm and death in hospital patients but also increases the healthcare expenditure every year. Little is known about the prevalence and associated factors of ME in the Emergency Department (ED) of hospitals in Malaysia. The objectives of this study were to determine the prevalence and factors associated with ME in an ED of a teaching hospital. A cross sectional study was conducted on patients who visited the ED of Hospital Universiti Sains Malaysia during office hours. Patient medication orders and procedures were observed and recorded using a specifically designed data collection form. Descriptive statistics were employed and data were presented as frequency and 95% CI. Multiple logistic regression was employed to determine factors associated with ME. Of a total 547 eligible patients, 311 were randomly selected for analysis. Ninety-five patients had at least one ME. The prevalence of ME was calculated to be 30.5%. The most prevalent ME was wrong time error (64.2%), followed by unauthorized drug error (34.7%), omission error (25.3%) and dose error (12.6%). Patients with cardiac disorders, respiratory disorders and gastric disorders were the most commonly affected. Factors associated with ME were number of medications [adjusted (OR) 1.91 (95%CI: 1.51, 2.41)], triage [adjusted (OR) 0.11 (95%CI: 0.04, 0.27)], gender [adjusted (OR) 0.50 (95%CI: 0.26, 0.93)] and time of patient visit [adjusted (OR) 0.55 (95%CI: 0.265, 1.12)]. The prevalence of ME in the ED was moderately high in this setting. Any intervention measures to minimise the MEs should take into consideration the associated factors identified.
