The prevalence of depression in Malaysia was 12.2% in 2018. Nowadays, depression has been one of the most common issues associated with pregnancy. Pregnant women are especially vulnerable because of the hormonal changes during pregnancy, childbirth, and following delivery. Not being able to participate in daily activities and lack of social support are major issues for pregnant women with depressive symptoms, which can lead to postpartum depression or even increased risk for developing depression. Therefore, the main objective of this study was to determine the prevalence of depressive symptoms among pregnant women in Terengganu and its associated factors. The multi-stage sampling was used in this two-phase cross-sectional study with a total of 422 pregnant women (n=33 in phase Ⅰ, n=389 in phase Ⅱ) aged 18 – 46 years, in second and third trimesters, recruited from 10 randomly selected Klinik Kesihatan (two in phase Ⅰ, nine in phase Ⅱ; one same clinic in both phases) in Terengganu. Anthropometric measurement comprised current weight, pre-pregnancy weight and height, while the body composition analysis was assessed using Bioelectrical Impedance Analysis. Structured questionnaires were used in both phases to collect socio-demographic data, medical history, utilization and perception on complementary alternative therapies (UPCATs), physical activity level (PPAQ-M), factors and symptoms of depression (EPDS). The descriptive analysis, simple and multiple logistic regression were used to interpret the data. Both UPCATs and PPAQ-M questionnaires were proven to be reliable and valid for this population with the overall Cronbach alpha (ɑ) of 0.831 and 0.744, respectively. For phase Ⅱ study, the majority of the respondents were in their third trimester (55.3%), Malay (98.5%), married and living with a spouse (98.2%), received education up to foundation (53.2%), unemployed (45%) and categorized in B40 income range (84.6%). 44% (n=171) of them were overweight (24.7%, n=96) and obese (19.3%, n=75) and have body fat percentage of 33.95 ± 5.83 %. Majority of the respondents engaged in the sedentary level of physical activity (71.2%, 1.19 MET-hour/day) in the form of occupational type (0.706 (81.84) MET- hour/week). Out of 389 pregnant women, 6.7% of them had depressive symptoms. The final regression model verified that the respondents who were married but not living with a spouse had higher odds (OR= 5.97, 95% CI: 1.10, 32.37, p=0.038) of getting depression compared to those who were married and living with a spouse. Based on the ROC curve result, the optimal threshold for BF% in identifying pre-pregnancy BMI ≥25kg/m2 was 33.9%. Overall, the prevalence of depressive symptoms among pregnant women in Terengganu was 6.7%. Depressive symptoms among pregnant women in Terengganu was significantly associated with their household living arrangement. Identifying the depressive symptoms especially in those who are in the high risk pregnant women population should act as one of the strategies of preventing and reducing depressive symptoms risk during pregnancy.
