Amiratul Athirah Binti Shamsuddin Genetic polymorphisms—Obesity susceptibility Obesity is a worsening phenomenon which escalates the risks of metabolic, cardiovascular diseases as well as cancer. The energy intake and expenditure, hunger and satiety are regulated by the leptin hormones and other adipokines. Leptin hormone encoded by Leptin gene is located at the chromosome 7q32.1. Leptin gene polymorphisms have been studied comprehensively in several populations in relation to body weight status and its comorbidities complication, but the evidence was indecisive. The purpose of the current study was to determine the association and molecular characterization of Leptin gene polymorphisms of overweight with obesity and its attributes among Malays in Kuala Terengganu, Terengganu, Malaysia. This case control study involved a total of 249 Malay subjects (101 healthy and lean adults with normal BMI as the control group and 148 of overweight and obese subjects). The anthropometrics data were obtained, and lipid profile analysis was conducted. PCR RFLP technique was performed to determine the genotype and allele distribution of Leptin gene polymorphisms. Single and haplotype association analyses were executed, and further fine mapping approach was required to locate the plausible precise causal polymorphisms. The genotypic and allelic frequencies of Leptin gene polymorphisms presented no significant difference between groups, G2548A (P = 0.93 and 0.74); H1328080 (P = 0.58 and 0.56); and A19G (P=0.72 and 0.38) respectively. However, there was a statistical significant difference between triglyceride level and genotypes of G2548A polymorphism (P = 0.016); between total cholesterol level and H1328080 genotypes (P= 0.027). In addition, multiple logistic regression analysis projected the women gender (adjusted OR= 26.27; CI= 1.06 - 1.25; P= 0.009), waist circumference (adjusted OR= 1.15; CI= 1.06 – 1.25; P= 0.001) and body fat percentage (adjusted OR= 1.43; CI= 1.20 – 1.70; P <0.001) were the independent risk factors for obesity. In addition, haplotype analysis demonstrated the AAG haplotype of H1328080, G2548A, and A19G respectively associated with obesity (OR= 8.89; CI= 1.59 - 49.78; P=0.002). The Linkage Disequilibrium value between SNPs were designated as between H1328080 and G2548A (r2=0.66), G2548A and A19G (r2= 0.65) and H1328080 and A19G (r2=0.98). The susceptible AAG haplotype was used as the reference haplotype to find possible variants. However, this study could not detect the plausible polymorphisms which contributed to body weight disorder might be due to reference haplotype was very rare in the Asian population. As a conclusion, anthropometric data such as waist circumference and body fat percentage may be referred to increased risks of obesity in Malay population.
