Ultrasound therapy (UST) and nerve and tendon gliding exercise are commonly used in physiotherapy clinical practice but studies have found uncertain evidence regarding the effect. Radial Extracorporeal Shock Wave Therapy (rESWT) is a novel non-invasive treatment for ICTS but there is very scarce literature on the usage of rESWT in ICTS. The objective of this study was to determine the effects of rESWT and UST on ICTS. This study has two phases; Phase 1 was a one-group experimental study involving 12 participants to determine the feasibility and acceptability of integrating rESWT into physiotherapy intervention for ICTS. The intervention was given twice a week for four weeks. The intervention's feasibility was assessed via feedback from the participants and the effects of pre and post interventions using paired t-test. Phase 2 was to evaluate the impact of rESWT and UST on primary and secondary outcome measures in patients with ICTS. A randomised controlled trial with a random sampling method was used. 60 participants were randomly allocated into three groups after fulfilling the inclusion criteria. The group received UST combined with nerve and tendon gliding exercise, rESWT combined with nerve and tendon gliding exercise and the Control group received only nerve and tendon gliding exercise, twice a week for four weeks. Primary outcome measures consisted of the Boston Carpal Tunnel Questionnaire (BCTQ) that includes Symptom Severity Scale (SSS) and the Functional Status Scale (FSS), and secondary outcomes includes visual analogue scale (VAS), grip strength, pinch strength (palmar and lateral), static two-point discrimination (2-PD) and thumb range of motion (ROM) especially the Metacarpophalangeal joint (MCP) and Interphalangeal joint (IP). The evaluation was performed at baseline, 2 weeks, 4 weeks, and 8 weeks. Data analysis was done using the mixed model ANOVA test. According to phase one results, the intervention of ESWT is feasible and acceptable for treating ICTS with no adverse effect reported. In phase 2, statistically significant time effects were observed for the SSS (p<0.001, F-stat (df) = 183.267(2) and FSS (p<0.001, F-stat (df) = 69.95(2). The same findings were also obtained for secondary outcomes such as VAS (p<0.001, F-stat (df) = 49.13(2), grip strength (p<0.001, F-stat (df) = 49.67(2), pinch strength (palmar) (p<0.001, F-stat (df) = 52.41(2), pinch strength (lateral) (p<0.001, F-stat (df) = 51.95(2), 2-PD (p<0.001 (df) = 12.15(2), and IP (p<0.001, F-stat (df) = 7.20 (2). However, MCP showed no significance (p=0.051, F-stat(df) = 3.08(2). The participants receiving rESWT combined with nerve and tendon gliding exercise showed greater reduction in the symptom and increase in hand function. This result implies that the rESWT is an effective modality in treating mild to moderate ICTS. rESWT has effectively reduced the severity and improved the hand functions in ICTS. The findings can be incoporated as a clinical practice guideline in the treatment for ICTS.
