As CHD rates continue to rise, the demand for CABG surgeries is also expected to increase, highlighting the need for effective post-operative care and patient education to enhance recovery outcomes. Traditional health education approaches often follow a standardized and generalized format, lacking individualized attention to patients' learning needs. However, research suggests that individualized patient education can improve patients’ outcomes during the early recovery period after CABG surgery. This study aims to investigate the specific learning needs of patients who have undergone CABG surgery during the first week (day 2) and third week (day 15) after being discharged from the hospital. Furthermore, the study aims to evaluate the effectiveness of the Individualized Self-Care Program (ISCP) in improving knowledge of self-care behaviour, enhancing the performance of therapeutic self-care behaviour, and reducing the severity of symptoms between the experimental and control groups of patients who have undergone CABG surgery at 1 and 3 weeks post hospital discharge. A Randomized Controlled Trial design that included a consecutive sample of 128 first-time CABG patients was adopted. Patients were assigned to the experimental group (n = 64) and the control group (n = 64). Patients in the experimental group were given individualized self-care programs post-discharge by the researcher at week 1 (Day-2) and week 3 (Day-15), while the patients in the control group received an ordinary health education. A paired t-test test indicated a statistically significant difference in the total number of learning needs identified between the post-test assessments in Week 1 and Week 3. Patients in the experimental group initially identified more learning needs during the first week (4.3±1.5). However, their knowledge had significantly improved by the third week, and they identified fewer learning needs (2.6±1.1). A Repeated Measure Analysis of Variance indicated that the individualized self-care program effectively improved CABG patients' knowledge of symptom management and performance of therapeutic self-care behaviours while reducing symptom severity. These outcomes were evaluated at multiple time points, specifically at the second week (days 9-10) and fourth week (days 22-23) post-hospital discharge. Individualized self-care programs delivered during the early recovery phase are vital for improving patients' self-care ability and performance and, as a result, lowering symptom severity. An educational intervention of this type would fill the gaps in the immediate post-discharge period. Furthermore, the current study shows that patients perceive learning needs in their home recovery, which change over time. Thus, the current study supports the need to implement individualized self-care programs tailored to CABG patients' needs and concerns in the early stages of recovery after discharge.
