The management in Indonesia has remained focused on the World Health Organization five pillars which cover education, diet planning, physical exercise, pharmacological interventions, and blood sugar monitoring, and it merely emphasizes on biopyshcosocial aspects only. To date the issues of lack of adherence to medication, lack of self-care, and lack of blood glucose control and low the quality of life of patients are still evident . It could be due to lack of holistic approach mainly on the spiritual aspect which influences patient insight and behaviour. There has not been management ha i ecifica dieced i e he a ie i i a a ec . Th hi d aims to determine the effectiveness of the Islamic Psychospiritual Guidance Module that has been constructed on adherence to medication, self-care, and HbA1C values, as well as the quality of life of Type II diabetes mellitus patients. Methodology: This research is a quasi-experimental study using the nonequivalent control group pre-test and post-test design. This study involved 180 respondents diagnosed with Type II diabetes mellitus registered as participants in the Chronic Disease Management Program (Program Pengelolaan Penyakit Kronis/PROLANIS) joining the National Health Insurance (Jaminan Kesehatan Nasional/JKN) organized by the Badan Penyelenggara Jaminan Sosial (BPJS) of Ponorogo Regency, East Java, Indonesia. The respondents were divided into two groups, the intervention group (90 respondents) and the control group (90 respondents). The research samples were determined using a purposive sampling technique. The intervention group was given three Islamic spiritual intervention sessions. The measuring instruments include the Morisky Medication Adherence Scale-8 (MMAS-8) questionnaire (adherence to medication), the SDSCA (Summary Diabetes Self-care Activity) questionnaire (self-care), and the WHOQol BREF questionnaire (quality of life). Meanwhile, Hba1C was obtained from the available records. The development of medication adherence, self-care, and quality of life variables was seen at 3 and 6 months after the intervention was given, while the development of HbA1C value was seen at 6 months after the intervention. Result: The results of the ANOVA test on time effect showed a significant difference in the average scores of each group, either on adherence, self-care, quality of life, and HbA1C. The intervention group had a significantly lower intervention effect score than the control group on medication adherence, but a significantly higher intervention effect score than the control group on self-care and quality of life variables. In the HbA1C variable, there was no significant difference between the HbA1C values of the intervention group and the control group. Conclusion: This study concludes that Islamic Spiritual Guidance Module is effective in increasing medication adherence, self-care, and HbA1C values, as well as quality of life in Type II diabetes mellitus patients. Furthermore, the spiritual aspect plays an important role in changing the behavior of Type II diabetes mellitus patients. This clarifies the importance of providing comprehensive and holistic nursing care, emphasizing not only the biological, psychological, and social aspects but also the spiritual aspect.
